All PERS forms are available for print, to download, or to have mailed to you. Please follow the instructions on this page to access the forms in the manner most convenient to you. Please note that all forms on this page are the most current version; so, if you have previously printed, downloaded, or received a PERS form, please reference the revision date on each form below to ensure you are using the most current version.

    Form 9A SRVC, Pre-Application for Service Retirement Benefits
    • Form 9A SRVC, Pre-Application for Service Retirement Benefits, must be submitted to initiate a claim for service retirement benefits.

    PHASE 2 - Refer toPERS Service Retirement Guide

    PERS will mail additional forms necessary for completing the service retirement application after receipt of the completed Form 9A SRVC.

    Disability Retirement Applicants
